After a doctor’s referral is received, a registered nurse visits the home to assess the patient's care needs and determines the family's ability to participate in care. A care plan is then established based on the nurse's observations, family needs, and physician's orders.
Yes. All services must be ordered by the patient’s physician. A referral consists of the patient’s address, phone #, insurance, primary diagnosis, orders for care, if skilled care is needed, reason for care, and the name, address and phone number of physician who will be following the patient and responsible for ongoing care.
Yes. If your child qualifies, he/she may have other services.
No. Your child may be able to attend school/daycare and have nurse accompany him/her.
At present, PNDV does not provide Speech Therapy or Occupational Therapy. We will announce when these services become available.
No. Although it is better to have all services provided by one agency, you are not required to do so. For example, you may have private duty nursing by one agency and attendant services with another.
